DAY OF SURGERY CHECKLIST
Forms (YOU WILL NEED TO BRING THESE THE DAY OF YOUR PROCEDURE)
Bring With You:
- Health insurance card(s),
- Picture identification card,
- Any applicable co-pays or fees. (If you are unsure of your co-pay, please contact your insurance carrier.)
Arrival Time:
A member of the Catalina Surgery Center staff will contact you the day prior to surgery to inform you of the final arrival and appointment time of your surgery. This may be somewhat different from your original information provided by your surgeon’s office.
Pre-Registration Information Forms
These forms may be reviewed at home and kept for your own personal record.
If you are having a surgical procedure, please remember that the anesthesiologist and any necessary pathology will be billed separately from your surgeon and the facility.
